Patient experiences of hospital discharge

Staff at Russells Hall Hospital shared our survey with patients and 85 people responded with their views about discharge from the hospital.

Summary

We worked with Russells Hall Hospital to find out patients experiences of discharge from the hospital. We found that while most patient felt understood in hospital, the were concerned with the complex discharge systems.

Key outcomes

  • The majority of patients felt satisfied with the discharge procedure.
  • Problems did arise with difficulties in understanding complex processes or failures in communication. 
  • 40% of patients told us they had no care plan on discharge.
  • Patients need to feel confident that they will get the advice and support they need to ensure their care needs are being met when they are discharged.
